Vital Care Infusion Services, LLC Privacy and Data Use Statement
At Vital Care Infusion Services, LLC (Vital Care), we recognize the importance of protecting an individual’s privacy and the privacy of healthcare data because trust and respect are among of our core values. We endeavor to ensure that everything we do as it relates to the data we collect and use to carry out our purpose and mission is compliant with regulatory standards and expectations.
Applicability and Scope
Vital Care is not a healthcare provider; we are a business associate performing claims billing and clinical consultation services to our franchisee pharmacies which are covered under the treatment, payment and operations allowance within Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules. Individuals or patients of the franchisees in our franchise network who need access to or have requests concerning their Protected Health Information (PHI) should direct them to the Vital Care franchisee that serviced them.
As a business associate with our franchisees, the following privacy statements and the commitments made therein, are limited to PHI which we use and collect:
We will comply with HIPAA Privacy and Security Rules and protect all patient PHI that we use or disclose with robust hardware, software, enhanced security measures, user authentication, and access protocols.
In collaboration and coordination with our franchisees, we will:
- provide patients with copies of their medical records, provide access, or an accounting of PHI disclosures upon written request and in a timely manner;
- release PHI to others only upon written authorization from the patient to do so unless the request is associated with purposes related to treatment, payment and operations or as otherwise permitted or required by HIPAA regulations;
- de-identify PHI as required and applicable to HIPAA regulations; and
- comply with data breach and applicable disclosure notification requirements to affected individuals and regulatory entities in a timely manner.

Other Information we Collect, Use, and Protect
